v0.9.0 Cancel Member cancellation reasons updated

Cancellation reasons for member are now as follows:

• Change in employment status
• Marriage
• Moving outside of the 175+ countries of coverage

Below cancellation reasons are not supported anymore:

• Departed from company
• Qualifying life event
• Moving to unsupported country of residence
• Hardship
• Moved to another provider
• Employee already has coverage
